NICOLE SWIATEK, DPT, BSMS, FAAOMPTS

Nicole Swiatek, DPT, BSMS, FAAOMPT, owner of Phoenix Physical Therapy and New York State Licensed Physical Therapist, has over 10 years of experience in outpatient orthopedics. She graduated from D’Youville College with a Bachelor of Science degree in Clinical Science and a Master of Science degree in Physical therapy. She completed her Doctorate of Physical Therapy (DPT) degree at D’Youville College in 2008. She completed fellowship training in orthopedic manual physical therapy at Daemen College in 2011 and is recognized as a Fellow of the American Academy of Orthopedic Manual Physical Therapists (FAAOMPT). She is a member of the American Physical Therapy Association and the American Academy of Orthopedic Manual Physical Therapists. She has also been recognized by Business First as a 2011 Healthcare Top 50 recipient, as well as a 2012 Thirty Under 30 recipient. She has completed numerous advanced training courses and is an adjunct professor at D’Youville College. Nicole is focused on providing her patients with hands-on, evidenced based treatment options to get them back to their pain-free lives. Nicole lives in Williamsville with her husband and spends most of her free time playing with her two young sons.

KIMBERLEE CARPENTER, DPT, MS, Cert. SMT

Kimberlee Carpenter, DPT, MS, Cert. SMT, is a New York State licensed physical therapist and has over 10 years of clinical experience in outpatient orthopedics. She graduated from D’Youville college with a bachelor of science degree in clinical science and a Master of science degree in physical therapy. In 2014 she completed her Doctorate of Physical Therapy (DPT) degree. Kim earned the distinction of becoming one of the first therapists in New York State to achieve advanced certification in spinal manipulation from the American Academy of Manipulative Therapy in 2010. She has also completed numerous advanced training courses in the areas of functional exercise, soft tissue management, and Mechanical Diagnosis and Treatment from the McKenzie Institute.  Kim was recognized as a “health care champion” by Buffalo Business First in 2016. As Clinical Coordinator of Education for Phoenix Physical Therapy, she enjoys mentoring students. In addition to her clinical involvement she is an adjunct professor at D’Youville College. She believes that the "hands on" approach when working with her patients in conjunction with an individualized exercise program promotes the best outcomes. Compassionate care is the foundation for her approach, and her main goal is to restore patients to their prior level of function quickly. She lives in East Amherst with her husband.

CHELSEA CIULIS, DPT

Chelsea Ciulis, DPT, is a New York State licensed Physical Therapist who received her Bachelor of Science in Exercise Science in May 2012 and completed her Doctorate of Physical Therapy (DPT) in May 2015, at the University of Buffalo. Chelsea has been a member of the American Physical Therapy Association for 4 years. She has completed advanced training courses in spinal manipulation and is an adjunct professor at DYC. Chelsea provides hands on treatments and gives individualized exercise programs to help improve patient’s quality of life and get them back to pain free movement and prior level of function.

KELLY MALSTROM, DPT

Kelly Malstrom, DPT, graduated from D’Youville College in 2015 with a Doctorate in Physical Therapy (DPT) along with an Advanced Certificate in Orthopedics. Kelly’s clinical interests are focused on treating her patients with a combination of hands on manual therapy treatment along with exercise in order to provide proper and effective treatment. Kelly enjoys the individual attention that she is able to provide her patients at Phoenix PT. She enjoys taking continuing education courses to provide her patients with effective and current evidence based care. Kelly has recently attended a spinal manipulation course in October 2015. She plans to take many other courses that focus on manual therapy approaches and corrective exercise. 

CRYSTAL MUZACZ, PTA

Crystal Muzacz, PTA, is a New York state licensed Physical Therapist Assistant who earned her degree in applied science with highest honors from Villa Maria College in May of 2016. As a recent graduate, Crystal is excited to begin her career at Phoenix PT and plans to expand her skill set by attending a McKenzie certified course for the lumbar spine in September. Prior to becoming a PTA Crystal served in the U.S Army and is a combat veteran. In her free time she enjoys being with her son and playing competitive roller derby with the Queen City Roller Girls’ travel team, the Lake Effect Furies.

AMANDA WALLER, PTA

Amanda Waller, PTA, is a New York State licensed Physical Therapist Assistant graduate with an Associate Degree in Applied Science from Villa Maria College in August 2015. Amanda is an American Physical Therapist Association member since October 2013. She joined Phoenix Physical Therapy in April 2016, and believes in the hands on philosophy of Phoenix Physical Therapy and enjoys assisting patients in their individualized treatment plans and progressing patients in their exercise programs to help them return to their previous level of functioning.
