HOW TO SCHEDULE AN APPOINTMENT ONLINE

Please use the link below to schedule your appointment. You will be asked to provide an email address or cell phone number and date of birth to complete the verification process.
 

EXISTING PATIENT

If you are an EXISTING PATIENT, please select your case, the office location and any therapist that you would like to see. Once you select the days and times that you would like, they will be sent to our office for APPROVAL. Once APPROVED, you will receive notification of your appointment. If we are unable to approve your selected appointment, you will receive a notification that that appointment slot is no longer available.
 

NEW PATIENTS

If you are a NEW PATIENT, please follow the steps to create a new patient chart. You will then select the office location and any therapist that you would like to see. Once you select the days and times that you would like, they will be sent to our office for APPROVAL. Once you are APPROVED, you will receive notification of your appointment. If we are unable to approve your selected appointment, you will receive a notification that that appointment slot is no longer available.
